Foxwood Springs is currently seeking a Clinical Liaison - LPN to join our team. The Clinical Liaison will be responsible for providing clinical support and education to patients, families, and healthcare providers. 

Responsibilities:

- Conduct patient assessments and provide education on various health conditions and treatment options

- Collaborate with healthcare providers to develop and implement patient care plans

- Communicate with patients and families regarding their healthcare needs and concerns

- Participate in case conferences and team meetings to discuss patient care and progress

- Serve as a liaison between patients, families, and healthcare providers

Requirements:

- Current LPN license

- Minimum of 2 years of experience in a healthcare setting

- Strong communication and interpersonal skills

- Ability to work independently and as part of a team

- Flexibility to work Monday-Friday and every other weekend on call rotation
