Oak Park Oasis is seeking a full time Psychiatric Rehabilitation Service Coordinator (PRSC) to whom the resident primarily relates for the coordination of services, programs, oversight of data collection and review of resident performance. Full or part-time, flexible hours available.

The candidate must meet the following qualifications:

• Minimum of one year long term nursing care experience working with the adult mentally ill population preferred
• BA Degree in Human Services field (including sociology, social work, special education, and psychology).
• Experience with caseloads, crisis intervention, documentation, care planning with involving families in the process, knowledge of the MDS, d/c planning, facilitating skills groups and psycho-social groups and problem solving
• Good assessment skills, interpersonal skills, and knowledge of MDS 3.0 along with basic computer skills, communication, advocacy skills and charting experience.
• Must be able to work in an interdisciplinary team environment
• Must be able to handle confidential data with professional discretion
• Position is 5 days/weeks 8 hours per days
