GENERAL DESCRIPTION AND PURPOSE OF THIS POSITION:
The Case Manager provides clinical leadership to residents and directs the clinical operations of residents to ensure regulatory compliance, nurture a person-centered and resident directed culture. The Case Manager provides each resident with professional licensed nursing assessments and evaluations and coordinates their care with other nursing staff. Also responsible for infection control, safety, wound management, falls, MDS/CAA process, etc.

PRIMARY DUTIES AND RESPONSIBILITIES OF THIS POSITION:
· Provides leadership to clinical staff from Households on specific areas of clinical accountability. Specific accountability may include infection control, safety, wound management, falls, MDS/RAI process or other areas as needed or determined.
· Guides the implementation of clinical policies and procedures which ensure safe, sanitary and efficient practices supporting professional nursing care standards; assures that documentation, medication administration, resident nursing care, safety and sanitation, personnel and staff development, MDS/RAI process, materials management, Quality Improvement and Resident Rights are in compliance with Oak Hills Living Center standards.
· Guides household staff to assure proper hiring, orientation, training, clinical competency, coaching, educating, counseling and performance evaluations are maintained in a manner supportive of team, as well as professional standards of practice.
· Guides household staff to support Oak Hills Living Center fiscal viability through efficient and effective use of resources in the household, including maintaining staffing and supply budgets within planned expenditures, maintains staffing patterns consistent with the philosophy, objectives and availability of resources.
· Guides the clinical component of continuous quality improvement in the household in support of a systemic approach to quality clinical care. Ensures households participate in Quality Improvement measurements and that appropriate follow up is taken to assure quality outcomes, performing professional audits and observations to confirm the competency of all household staff specific to the Nurse Mentor's areas of clinical accountability.
· Fosters collaboration, consensus and negotiation to address problem solving in the household , assures that clinical policies and procedures of the household are maintained, interprets and communicates same in the Household as needed, with an emphasis on resident, family or designated representative.
· Maintains positive working relationship with medical care providers in support of quality care in households.
· Supports meeting all resident and household needs by actively assisting (to the extent of cross training and certification) in all tasks to meet identified needs.

QUALIFICATIONS, KNOWLEDGE, SKILLS AND ABILITIES:
· Must meet regulatory requirements for licensure as registered nurse in state of Minnesota.
· Experience in geriatric nursing is desirable.
· Three to five years on the job experience as nurse in skilled health care facility is preferred.
· Must have demonstrated mentorship qualities, supervisory skills, interpersonal relationship · skills, and knowledge of federal and state regulations governing clinical care, and nursing practice.
· Commitment to person centered and resident directed care is essential.

Salary: Starting at $32.00 - $34.00 per hour

Schedule:
Full-time AM Shift - Monday - Friday from 8am - 4:30pm*
*On-call rotation every 8th weekend
