O'Neill Healthcare is Hiring a Wellness Coordinator AL Near Lakewood, OH

O’Neill Healthcare Lakewood is currently seeking an Assisted Living Wellness Coordinator to oversee the Assisted Living unit. This is a nurse manager position requiring an LPN licenses in the state of Ohio.

Our six west side facilities provide seniors with the diverse, personal, and quality care they need. Our facilities and services range from Assisted Living, Skilled Nursing, Hospice, Rehab Therapy, and Dialysis.

Primary Responsibilities include:

1. Total responsibility for quality resident care.
2. Documentation including but not limited to:
a. Medical Records
i. New Admissions/Discharges
ii. Assessments
iii. Progress Notes
b. Staff in servicing and training
3. Education and training for residents and staff.
4. Family/Visitor intervention.
5. All other duties as directed.

Qualifications:

1. A graduate of an accredited nursing school and licensed to practice nursing in the state of Ohio
2. A minimum of one-year prior geriatric experience
3. Current CPR certification.
4. Must be able to communicate with staff, residents, their families, and visitors.
5. Must be in good health, no lifting restrictions, neat and clean.
6. Must be willing and able to learn and possess basic management, organizational, and communication skills.
7. Must project a positive attitude
