Job Description

Reporting directly to the NRU Director, the Clinical Provider will provide harm reduction oriented, low-barrier care in a street or home setting for people experiencing homelessness as well as those with substance use and mental health challenges. This will involve screening, triage, evaluation and treatment, referrals to other care providers and prevention counseling as appropriate for substance use disorders and health issues related to homelessness and substance use like Hepatitis C, HIV prevention, overdose prevention, wound care and mental health.
This may include:
  • Establish and maintain therapeutic relationships with Community Members and other care providers and collaborate with Community Members and providers to identify and overcome barriers to treatment
  • Triage of acute complaints, provide care as appropriate and connection to a level of care appropriate to severity of condition
  • Provide health education and harm reduction counseling related to substance use and other health issues
  • Assess and manage wounds related to substance use and chronic disease, including performing minor procedures, dressings, and referral to higher levels of care as needed.
  • Perform screenings for behavioral health conditions and referral for appropriate care
  • Dispense syringes as allowed under the NYSDOH Expanded Syringe Access Program
  • Perform Hepatitis C screening, education about treatment and connection to care
  • Perform HIV screening, education, and connection to treatment
  • Assist in developing clinical partnerships and liaise as needed with local community health care and service providers like pharmacies, opioid treatment providers, hospitals, clinics, DOHMH HEAT team, and syringe service programs to assure that community members get needed treatment.
  • Document care provided according to agency, New York State and Federal requirements.
  • Order and review appropriate labs
  • Link Community Members to services that may benefit their treatment goals, including but not limited to primary and specialty healthcare, harm reduction counseling, behavioral health counseling and other supportive services.
  • Perform screenings for substance use disorders and starting immediate treatment when appropriate (i.e. buprenorphine for opioid use disorder) or referral for appropriate care
  • Administer vaccines as part of planned immunization efforts (i.e. Hepatitis A and B, COVID, Flu)
  • Administer medications, including psychiatric medications
  • Dispense naloxone


Minimum Qualifications

1. A valid New York state license as a Registered Nurse, a baccalaureate degree in Nursing from an accredited college and a master's degree in Nursing, Public Health or a related field, plus four years of recent experience in public health or hospital nursing or a related field; at least 18 months of which must have been in an administrative, managerial, executive, consultative or educational capacity, or in supervision of professional public health nursing personnel working in any of the above mentioned areas; or
2. Education and/or experience equivalent to "1" above. However, all candidates must possess a valid New York State license as a Registered Nurse, and a baccalaureate degree in Nursing from an accredited college; and must possess the 18 months of specialized experience as described in "1" above.


Preferred Skills

- RN licensure to practice in NYS, with experience in family medicine, internal medicine, primary care, psychiatric or substance use treatment - 2-3 years prior clinical experience working with people experiencing homelessness, substance use disorders, living with mental illness or who are justice-involved. - Knowledge of treatment for substance use and mental health disorders - Commitment to a person-centered and harm reduction approach to care - Flexibility and comfort providing care in street settings - Good communication and collaboration skills - Bilingual English/Spanish preferred
