Purpose of Position
Under the direction of the Supervisor/ADN/DON, the Head Nurse of the Ambulatory Care Department is responsible for planning, organizing, directing, coordinating, and evaluating the quality of care rendered in the clinic. As the Head Nurse, she/he has the authority and responsibility of managing the unit to guarantee quality care for every patient seen in clinic and is held accountable by the use of the nursing process. She/he is responsible for the implementation of the Nursing Department philosophy and objectives of Nursing Care through the use of the department’s policy and procedures.
Functional Duties/Responsibilities:
ö Reports directly to the Supervisor/ ADN/DON.
ö Performs all staff nurse responsibilities.
ö Checks for absent and late employees and monitors the computerized sign-in and out sheet documentation.
ö Performs initial assessment and re-assessment of patient receiving care in the clinic and assists MD with all examinations.
ö Delegates staff to perform the following functional assignments such as:
(a) Taking vital signs, performing phlebotomy, EKG
(b) Preparing patients for various procedures and proper consents.
(c) Checking code cart.
ö Makes staff assignments; give each staff member the briefing necessary for the care of patients in the assigned clinic.
ö Supervises medication treatment and diagnostic procedures administered or performed by staff.
ö Conducts interdisciplinary referrals or conferences as the need arises with therapist, social worker, dietitian, and physicians and other health team members.
ö Collaborates with administrative staff to enhance patient flow and ensure smooth and efficient daily operations.
ö Initiates or delegates assignments for the requisitions for ordering of medications, surgical supplies.
ö Supervises:
(a) Updating of medications
(b) Removal of recalled or outdated drugs.
(c) Care of special drugs such as narcotics, (if indicated).
(d) Care of medication cabinets and medication refrigerator.
ö Enforces staff to observe:
(a) Safety measures
(b) Infection Control practices
(c) Care and economy of supplies and equipment
(d) Managing of time
(e) Privacy and confidentiality for patients
ö Requests and receives reports from staff. The conditions of patients and significant occurrences observed.
ö Educates and teach based on patient diagnosis and need.
ö Assists and supervises care of critically ill, cardiac arrest, fire and disaster events.
ö Has knowledge of immunization and keep abreast of DOH changes.
ö Is able to plant PPD and interpret results as per guidelines.
ö Knowledgeable in the use of glucometer, maintenance and quality control.
ö Is aware of fire safety/evacuation plan.
ö Is aware of disaster plan.
ö Is aware of hazardous waste management program.
ö Evaluates patient care problem and initiates solutions.
ö Make verbal and/or written reports of problems encountered as well as outstanding occurrences on the unit to DON/ADN.
ö Conducts nursing care/surveillance rounds for clinic activities with staff.
ö Makes rounds or visits with patient to determine satisfaction with care.
ö Initiates and conducts patient and staff teaching as the need arises.
ö Conducts meeting with staff.
ö Observes and follow-up on all cases of suspected child/elder abuse.
ö Assigns patients to herself for active care as the need arises.
ö Participates in patient discharge planning with other members of the health care team.
ö Ensures a safe and secure environment free of hazards to reduce the risk of injury to patients, visitors, and staff.
ö Participates in team building and problem solving on their unit and hospital-wide.
ö Ensures staff is educated on new and continued surgical procedures and protocols as it relates to the clinical subspecialty.
ö Keeps anecdotal records of significant occurrences on the unit and behavior and performance of staff.
ö Evaluates staff performance with DON or ADN annually.
ö Conducts and participates in employee disciplinary process.
ö Coordinates nursing care conferences and lectures.
ö Attends committee meetings and participate as a member.
ö Conducts meetings with staff to communicate the implementation of administrative policies and care innovations, to correct weakness of patient care and staff performance.
ö Assists in the development, implementation and data collection of ambulatory care/unit based performance improvement projects.
ö Assists with equipment and supply inventory.
ö Participates in pilot studies of nursing research.
ö Participates in improving organization performance activities, whether it is in the realm of data collection, data analysis, data interpretation, planning or implementation of the plans in response to the outcomes of IOP.
ö Maintains clinical competency by attending continuing education programs.
ö Performs all related duties as assigned by the ADN/DON.
ö Maintains a level of cultural competence.
ö Knowledge of interpersonal/team building methods.
ö To be aware of the population specific patient management for the area which they work.
ö Understands policy and procedure related to Limited English Proficiency (LEP).
Position Knowledge
ö Assumes responsibility for personnel continuing education and development needs.
ö Keeps current in knowledge and practice of specialty clinic patient care and education.
ö Keeps current in established standards in diabetic practice as specified by the American Diabetic Association and the American Association of Diabetes Educators.
ö Keep current in knowledge and understanding of equipment and supplies used in diabetic management.

Minimum Qualifications
For individuals holding an unrestricted registered professional nursing license BEFORE July 1,
2020 in New York State, another state or United States territory:
1. A valid New York State license and current registration to practice as a Registered Professional Nurse
in New York State; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life
Support (BLS) through the American Heart Association (AHA); and
3. One (1) year of experience as a registered nurse.
For individuals issued a registered professional nursing license on or AFTER July 1, 2020,
based on expected implementation of Chapter 502 of the Laws of 2017 and Chapter 380 of the
Laws of 2018:
1. A valid New York State license and current registration to practice as a Registered Professional
Nurse in New York State; and
a. A Bachelor’s of Science degree in Nursing or higher (or other nursing degree, as identified by NYS
Education Department including Master of Science in Nursing, Doctor of Nursing Practice, Doctor of
Philosophy Degree in Nursing or Doctor of Nursing Science) from an accredited college or
university, registered with the NYSED; or
b. Obtains a Bachelor’s of Science Degree in Nursing or higher, as described in “a” above, within ten
(10) years of the date of initial licensure; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life
Support (BLS) through the American Heart Association (AHA); and
3. One (1) year of experience as a registered nurse.
