Overview:
We are seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will play a vital role in providing support and assistance to individuals and families in need. This position requires expertise in various areas, including senior care, working with people with chronic end of life diseases, clinical counseling, group therapy, individual counseling, mental health counseling, pediatrics, medical records, diagnostic evaluation, and crisis management.
Duties:
- Conduct comprehensive assessments of clients' needs and develop individualized care plans
- Provide counseling and support to individuals and families facing various challenges
- Collaborate with healthcare professionals to ensure coordinated care for clients
- Advocate for clients' rights and access to resources and services
- Assist clients in navigating complex systems such as healthcare, housing, and social services
- Maintain accurate and up-to-date documentation of client interactions and progress
- Facilitate group therapy sessions and support groups
- Provide crisis intervention and management as needed
Requirements:
- Bachelor's or Master's degree in Social Work or related field
- Valid state licensure or certification as a Social Worker
- Strong knowledge of social work principles, theories, and practices
- Experience working with diverse populations and addressing complex social issues
- Excellent communication and interpersonal skills
- Ability to work independently as well as part of a multidisciplinary team
- Proficiency in medical record documentation and case management software
