Position:Registered Nurse - Shift Nurse

Days: Monday – Sunday

Time: open ( preferred 10 am - 6pm)

Location: Berkeley, CA

Position Overview:

Under the general direction of a qualified Case Manager , the RN will assists in the nursing process by providing nursing care as per phyccian order and Plan of Care .

Primary Job Responsibilities:

Provides direct nursing care according to the Plan of Care and Physician orders.

Utilizes physical and psychosocial assessment skills, perform multi-system assessments, and identifies current and potential care needs

Carries out physician orders for treatment and visit frequency per plan of treatment

Organizes and completes patient treatment and tasks per established care plan

Implements and updates care plan in collaboration with the Clinical Manager and Primary Care Clinician to reflect progress toward goals and outcomes

Coordination care among team members and documentation in the clinical record per policy

Accurately documents ongoing assessments, clinical findings, skilled care, and plan for the next visit according to the established care plan

Qualifications:
Education
Graduate from an accredited vocational school of nursing or equivalent.

Experience

Minimum 1 year of professional experience in home health required.

Direct patient care experience within one year.

Home health or community health experience preferred.

Skills and Knowledge

Basic competencies in the fundamentals of nursing and pharmacology are required.

Computer skill is a must

Demonstrated skills in effective written and verbal communication.

Ability to recognize the needs and concerns of diverse groups of people.

Ability to maintain harmonious constructive working relationships with RN Case Manager, Families, and Doctors office.

Ability to use and give the instruction of adaptive equipment.

Must be able to handle sensitive issues, conflict with or among others, think critically and to effectively solve unique problems as they arise or identify when to consult a supervisor.

Must be able to deal with a challenging work environment with time demands and occasional conflicting priorities.

Job Type: Full-time

Pay: From $45.00 per hour
