This is a full-time, salaried position, Mon-Fri 8:30 am to 5:00 pm.
Responsibilities:
- Manages and directs a team of Field Staff ensuring safe, effective, and appropriate home care services.
- Receives referrals, determines services required, and Agency’s ability to meet needs. Oversees assignment of appropriate clinicians to cases.
- Internal Case Management for your team.
- Meets with Case Managers/Clinical Staff on a regular basis to provide guidance and information related to specific issues. Provides direction to teams to assure that client needs are met and services are provided according to the plan. Assists clinical staff in establishing priorities, setting goals, and evaluating progress toward goals
- Attends and assists in conducting case conferences and other clinical meetings to facilitate coordination of care.
- Reviews monthly team members’ productivity information.
- Coordinates 24-hour Agency coverage by Registered Nurses. Evaluates the quality of on-call services.
- Rotates in administrative on-call for field staff guidance as needed.
- Provides support and direction to Agency staff, other health care professionals, clients, and families related to appropriate and available health care resources.
- Educates Agency staff on clinical services, policies, and procedures as needed.
- Ensures accuracy, completeness, and timeliness of clinical documentation in accordance with Agency policies and procedures.
- Monitors open and closed charts regularly, and participates in the ongoing QA of clinical record reviews.
- Follows Agency guidelines for disciplinary actions. Documents all disciplinary actions in accordance with Human Resource policies and legal guidelines.
- Provides on-site supervision per Agency policy and as needed to determine staff competency and respond to educational and developmental needs.
- Conducts timely performance evaluations in accordance with Agency policy.
- Assists in the screening and interviewing of new Agency personnel and makes recommendations to the Director. Assists in the orientation of new personnel.
- Reviews OASIS data collection to assure accuracy and consistency with requirements. Communicates with clinicians and provides training as indicated to achieve compliance with collection and reporting timelines.
- Provides leadership to team and support staff in identifying Agency/client needs and opportunities for quality improvement.
- Graduate of an accredited school of professional nursing.
- Current license as a Registered Nurse in AZ.
- Minimum of two years of Home Health experience with a thorough knowledge base of OASIS, CMS guidelines, and State Regulations.
- Ability to multi-task in a fast-paced environment.
- Knowledge of Microsoft Office Suite; proficiency with technology.
- Minimum of one year of supervisory experience.
- Excellent verbal and written communication skills and strong interpersonal skills.
- Has a knowledge base of the scopes of practice of various disciplines including RN, LPN, PT, OT, ST, and CNA's.
- Licensed driver with automobile insured in accordance with the Agency and state requirements.
- Starting salary depending on experience.
