*Location:** Los Angeles, CA, various settings including clinic, care center, home, virtual, SNF, etc.
*Responsibilities:**
- Act as a pivotal member of the Interdisciplinary Team (IDT), contributing to patient assessments, care plans, and service determination requests.
- Lead personalized care for a panel of participants in collaboration with medical providers, ensuring progress on care plans.
- Deliver nursing care across different settings including primary care, medication administration, wound care, and oxygen therapy.
- Respond to emergency situations within the scope of practice and initiate appropriate therapies.
- Educate participants, caregivers, and staff on care plans and response protocols.
Qualifications Required:
Associate degree or higher in Nursing
- Minimum 4 years of experience in wound care and as a Registered Nurse (RN) of the following:
Geriatric Care
Senior Care
Geriatric/LTC
- Valid American Heart Association Basic Life Support (BLS) certification. - Valid RN license in California. - Fluent in English and Spanish.
*Preferred:* - Valid Alzheimer's certification. - Experience working in a PACE model.

- Pay Range: $49-61/hr.
Eligible for $15,000 sign-on bonus (after 90 days).
