Job Functions:

• Perform and document resident assessments upon admission and on an on-going basis through discharge.
• Complete all required nursing documentation accurately and timely.
• Participate in “hand off” report.
• Observe, record and report to supervisor or physician in a timely manner, any changes in the resident’s condition.
• Participates in the development and updating of the Interdisciplinary Plan of Care within the required timeframes.
• Implement the plan of care, using appropriate nursing interventions according to CareMeridian’s Policies and Procedures and regulatory guidelines.
• Administer and document medications and treatments as ordered, and observe and document the resident’s response.
• Transcribe and carry out physicians’ orders.
• Initiate emergency procedures per CareMeridian’s Policy and Procedure, if needed.
• Assist physicians as necessary.
• Actively participate in Infection Prevention Program.
• Maintain accurate infection tracking records.
• Participate in Interdisciplinary Team Conferences, as required.
• Provide resident and/or caregiver education.
• Communicate and cooperate with the interdisciplinary team to ensure that all therapeutic interventions are carried out timely.
• Attend and participate in continuing education programs per CareMeridian’s policies and regulatory guidelines.
• Complete and submit Incident and Accident Reports as needed.
•  Follow Safety Policies and Procedures at all times to include participation in fire and emergency management drills.
• Maintain the confidentiality of all residents’ data, per HIPAA requirements.
• Participate in the Performance Improvement Program.
• Use equipment per manufacturers’ instructions.
• Participates in coaching and supervising CNA’s daily duties and responsibilities.

Qualifications:­

• Current LPN/LVN licensure with the State Board of Nursing in the State of Nevada
• Current CPR in compliance with the American Heart Association standards
• One-year of nursing experience
• Negative TB/PPD or Chest X-Ray within the last 12 months
• Successful completion of the skills assessment specific to the areas of experience
