Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals in need, helping them navigate through various challenges and connect them with the appropriate resources. Responsibilities:
- Conduct comprehensive assessments to determine clients' needs and develop individualized care plans
- Provide counseling and support to clients, utilizing motivational interviewing techniques and crisis intervention strategies
- Collaborate with other professionals, such as social workers, mental health counselors, and addiction counselors, to ensure holistic care for clients
- Assist clients in accessing community resources, such as housing assistance, healthcare services, and employment opportunities
- Monitor clients' progress and make necessary adjustments to their care plans as needed
- Advocate for clients' rights and ensure they receive appropriate services
- Document all interactions and maintain accurate records of clients' progress

Qualifications:

- Previous experience in case management or a similar role is preferred
- Strong knowledge of behavioral health principles and crisis management techniques
- Excellent communication skills, both verbal and written
- Ability to build rapport with diverse populations and establish trusting relationships
- Proficient in conducting assessments and developing care plans
- Familiarity with community resources and the ability to connect clients with appropriate services
- Strong organizational skills and the ability to prioritize tasks effectively
