NTSOC is looking for a compassionate nurse to provide high quality nursing care for our pediatric and adult clients to improve their quality of life at home.

Essential Functions:

· Provide one on one care to a single client in their home setting and while in the community.

· Implements the client’s physician ordered plan of care and nursing interventions while adhering to agency policies.

· Perform assessments to monitor the health status of the client.

· Notify the physician and Field Nurse Case Manager of any changes in the client’s condition.

· Coordinate care with the Field Nurse Case Manager and other members of the client’s multidisciplinary care team.

· Administer medications and treatments as ordered.

· Document performed interventions and treatments.

· Assists clients with activities of daily living such as bathing, toileting, dressing, and grooming.

· Collaborate with the care team on implementation and updates to the client’s plan of care.

· Monitor client progress toward home health care goals.

· Educate the client and family regarding the plan of care and nursing care needs.

Minimum Education: Graduate of an Accredited Nursing Program required.

Minimum Experience: 1-2 years home health care experience preferred and 1 year of nursing experience in a health care setting preferred. Strong communication skills. Strong computer skills.

Licenses/Certifications: Colorado RN license in good standing with the Department of Regulatory Affairs. BLS required. Up to date car insurance and driver license will be required.
