Welcoming and loving oriented FQHC - Health Center seeks a Family Nurse Practitioner / FNP in Fitchburg, MA!

- Found in 2002, the QHC is a team setting that provides compassionate care regardless of income status This health center has 5 locations, 25 providers, and 400 team members with solid tenure with many team members, 15 yrs to 20 years
- Competitive salary, depending on experience incentive bonus program

- 3 weeks vacation, up to 40 hours of sick time, 1 week CME Retirement with 3% Match
- Full time, Monday-Friday 8am-5pm
- Team based mindset with collaborative environment
- FNP will work in a cardiovascular setting assessing patient's physical and psychosocial health/illness status through taking history and physical examination
- Nurse Practitioner will diagnose, manage, and refer patients with patients with acute, episodic, or chronic illness
- Board Certified Family Nurse Practitioner
- 2 years of experience as a Family Nurse Practitioner / FNP
- Active MA License
