Who We Are: Notre Dame Health Care Center, deeply rooted in the heritage the Sisters of Notre Dame de Namur, is committed to the physical, spiritual, and emotional care of our residents. Located off the shores of Lake Quinsigamond within the woods of the Worcester Land Trust, our Notre Dame Hospice program seeks qualified candidates committed to providing high quality care and compassion to our residents and clients. • Follow and maintain HIPAA guidelines and regulations with respect to all patient information. Protect the security of medical records to ensure that confidentiality is maintained.
•Visits hospice patients and their families identified as appropriate for spiritual care, completes spirituals assessment, and provides direct nondenominational spiritual counsel, prayer and support.
•Co-ordinates with community clergy as needed
•Participates in developing the interdisciplinary plan of care.
•Assists other core members of the Interdisciplinary Team to understand the significant spiritual concerns related to the patient/family response to dying.
•Serves as a liaison to and resource for community clergy in dealing with hospice patient/families and coordinates ritual/sacramental ministry with community clergy as appropriate and requested.
•Makes direct contact with clergy for patients and/or families who need assistance in contacting a clergy person.
•Assists in developing and providing educational programs for staff and the community.
•Maintains contact with appropriate educational spiritual care groups and professional organizations for ongoing education and support
•Represents the organization in a positive and professional manner in the community.

Requirements
• Certification in Educational and Pastoral Ministry or related degree with equivalent ministerial experience.
• Experience in a ministerial setting, preferably involving work with dying persons and their loved ones.
• Understanding of and commitment to the hospice philosophy of care.
• Openness to working with diverse religions and ethnic populations.
• Ability to work effectively as a member of the Interdisciplinary Team
• Must maintain a valid drivers license and dependable transportation.
• Expected to participate in appropriate continuing education as required.
• Possesses excellent verbal and written communication skills.
