Position: COORD - OUTREACH/REHAB
POSITION SUMMARY: Under administrative supervision of the Physical Medicine Manager, Coordinator manages, plans, and organizes the Rehab patient’s admission process. He/she works collaboratively with the team to optimize patient outcomes and maximize reimbursement for the organization; Assists with the accurate processing and collection of clinical data and subsequent submission to CMS via the UDR-MR software; Participates in developing and implementing marketing strategies that parallel the admissions process in collaboration with the Inpatient Rehabilitation Manager. Also demonstrates problem solving skills, and collaborative skills to work with other healthcare teams, including utilization management activities and promotion of high-quality cost-effective care in a collaborative manner.

Job Duties/Responsibilities:
  • Coordinates the referral development system designed to facilitate referrals to the Unit.
  • Conducts thorough pre-admission assessments of rehab candidates from phone inquiries, faxed referrals and onsite assessments when appropriate. Makes acceptance recommendation to Medical Director and/or Physical Medicine Manager.
  • Collaborates with in-house staff to offer rehabilitation services to patients at Northwest Texas Hospital who qualify. Coordinates with staff to transfer patients promptly to the Rehabilitation Unit to insure optimal care.
  • Obtains insurance verification and authorization to ensure that the rehabilitation services are reimbursable.
  • Contacts insurance representatives/case managers to obtain authorization for services. Maintains concurrent review documentation for medical necessity. Is accountable for final editing of all Patient Assessment Instruments (PAI’s) for all patients to insure timely transmission of data.
  • Identifies trends and collaborates on performance improvement activities.
  • Participates in community relations’ activities, including relationships with physicians, healthcare facilities, case managers, payors, and other post acute services and liaisons.

Qualifications:
EDUCATION, TRAINING, EXPERIENCE AND LICENSES/REGISTRATIONS REQUIRED:
  • RN, PT, OT, ST/CCC, or MSW licensed to practice in the state of Texas
  • Associate degree in nursing required Bachelors degree in nursing (other healthcare-related field) preferred or PT, OT, ST/CCC Masters degree or
  • Minimum of five years clinical experience
  • Minimum of two years clinical rehabilitation experience
  • Knowledge or experience of Medicare Rehabilitation regulations strongly preferred
