PRIMARY DUTIES AND RESPONSIBILITIES (may include but are not limited to the following):

• Conduct brief screenings to evaluate the needs of adults, adolescents, and children for behavioral health intervention, case management, or ongoing mental health or addiction treatment. Provide referrals for more intensive services where appropriate. 
• Provide short-term behavioral interventions to engage and motivate clients to actively manage their care. Conduct follow-up as appropriate based on assessment and treatment. Provide crisis management for clients as needed; make linkages for interventions as appropriate. 
• Participate in ongoing training in behavioral health issues and provide consultation and training to primary care and dental clinic staff in mental health and behavioral medicine topics including communication skills, brief interventions, counseling skills, motivational interviewing, medication use and misuse, substance abuse, wellness, rehabilitation, recovery and self-sufficiency. 
• Establish and maintain a current listing of community linkages to meet the needs of the target population. Develop and maintain collaborative working relationships with community providers. Initiate linkages disseminate information and make referrals to the appropriate services as needed. 
• Participate in regular multidisciplinary case conferences with medical providers as needed. Manage a panel of patients in collaboration with other behavioral health staff

EDUCATION, FORMAL TRAINING, AND EXPERIENCE (minimum requirements)

• Master's prepared Social Worker LMSW / LLMSW or LPC 

CERTIFICATIONS, LICENSES (minimum requirements)

• Licensed as a Master's Social Worker in the State of Michigan (will consider Limited Licensure)
