Residency Special Features: Northwest Medical homes (formerly Springfield Family Physicians) is the largest independent primary care organization in Lane County. Our three clinics are located in the Eugene/Springfield area and are staffed with 1-2 clinical pharmacists per 5-10 providers. We are recognized as a Tier 5 Patient-Centered Primary Care Home (PCPCH) which demonstrates our commitment to innovative, team-based care. This PGY2 residency will focus on providing the most advanced model of ambulatory care. Our pharmacy department sees patients with behavioral health and medical providers in a co-consult model, and independently under collaborative drug therapy management agreements (CDTM) for a variety of disease states. Additional required learning experiences include administration/leadership, teaching, business/new service development, community health at the local Federally Qualified Health Center (FQHC), population health, and a longitudinal ambulatory care staffing experience in all three clinics. Residents have the opportunity to pursue a teaching certificate through a program co-sponsored by the two local colleges of pharmacy, as well as precept a PGY1 resident or pharmacy student. Projects include teaching during our staff meetings, a longitudinal research project focusing on creation or evaluation of an ambulatory care service, and updating or creating a CDTM. Upon graduation our resident will have the knowledge and skillset to create and justify their own ambulatory care position in a variety of settings. **Start date of the residency can be flexible based on the needs of the resident
