AWV & Quality Department Medical Assistant
Position Summary: The Annual Wellness Visit team member works in collaboration with the primary care providers, staff and the clinical pharmacists at Northwest Medical Homes. Primary responsibilities include identification of patients with Medicare that are appropriate for Annual Wellness Visits (AWV) and execution of the detailed processes involved in independently completing AWVs.
This position may also assist the quality team, clinical pharmacists, and medical staff with completion of other clinical and non-clinical tasks as needed.
Essential Functions :
  • Strong attention to detail; ability to find information and summarize it quickly and precisely for providers and patients.
  • Ability to have difficult conversations with patients while maintaining tact and professionalism.
  • Proficiency in Annual Wellness Visit guidelines and quality metrics.
  • Ability to work independently and as a team.
  • Familiarity with medical complexities of patient and patient charts (POLST, advance directive, medications, procedures, history, etc.).
  • Ability to educate patients on preventive measures and screening procedures.
  • Ability to navigate quickly and accurately in patients' charts.
  • Ability to multitask; requires obtaining information from patients while looking at their charts to add to documentation.
  • Ability to take direction well and adapt quickly to changes.
  • Assesses healthcare needs that require additional follow up by primary care provider and assists patients in scheduling appropriate follow-up.
  • Be knowledgeable of community resources and assists patients in connecting with those resources.
  • Serve as patient advocate and assist in identification and improvement of service delivery.
  • Identify and address open quality gaps with patients and work collaboratively with patient towards closure of those care gaps.
  • Participate in regular team meetings, huddles, staff meetings and quality improvement projects to improve patient care.
  • Serve as a mentor and coach to others trying to improve the quality of care we provide
