Sign on bonus offered to eligible experienced RN's $15,000 This is a FT position working in Case Management at Northwest Medical Center in Tucson, AZ What you'll do:The Case Manager RN coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Coordinating services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.Assists in development, planning, coordination and administration of the activities of clinical review, discharge planning, resource utilization and utilization review.
