Description

Job Summary: The MAT Care Coordinator will coordinate and monitor patients through the phases of Medication Assisted Treatment plan in collaboration with the medical team. They initiate the patient intake process. Conduct screenings/assessments upon intake, at six months and at discharge of the program.

Role & Responsibilities

  • Maintains working knowledge of the company, Northwest Health Services, and the MAT program, policies, procedures, and standards including the implementation of social/function-oriented model of patient care.
  • Provide case management duties including, identifying and reducing barriers to care for patients.
  • Provide ongoing management, education and support of patients in all phases of treatment.
  • Track internal and external referrals of patients utilizing integrated health services.
  • Prepares patient record information for provider referrals and communicates significant information to other health care providers/entities as required.
  • Support the patient in accessing ongoing primary care
  • Ensures the delivery of compassionate quality of care approach to patients and ensures their care needs are met.
  • Communicates with other disciplines to evaluate patient responses to interventions, suggests alternative approaches, and revises and updates plans of care to reflect progress towards goals.
  • Promptly reports problems, significant changes in condition, injuries, and family dissatisfaction to supervision and provides timely responses to inquiries of same.
  • Assists team to effectively manage patient issues and interventions.
  • Responds to questions and issues identified by patients and families to determine satisfaction with services.
  • Provides timely and appropriate charting, documentation, and record keeping.
  • Attends meetings and other pertinent educational programs as approved.
  • Other duties as assigned.

Qualifications


Education & Experience

  • High school diploma or equivalent.
  • Experience in care management or social service area for 2 years preferred.

Required Knowledge, Skills, and Abilities

  • Must be able to talk, see, react, stand, kneel, walk and sit for long periods of time
  • Possess cognitive skills necessary to meet the primary responsibilities outlined
  • Must demonstrate patience, compassion, and good communication skills
  • Must respect and maintain confidentiality at all times
  • Ability to initiate, grow, and maintain positive working relationships with staff and other organizations
  • Maintain professional appearance and demeanor
  • Ability to problem-solve
  • Exhibit effective leadership skills
