CLINICAL QUALITY MANAGER

Northwest Florida Community Hospital (NFCH), in Chipley, FL, is seeking a Full Time experienced, energetic, and dedicated Clinical Quality Manager to lead the chronic care staff, and help provide exceptional care for patients in conjunction with the NFCH Clinic Medical Providers and staff.

The successful candidate will be responsible for the following duties:

  • Manage the chronic care coordinator staff
  • Monitor and analyze data on various quality measures and identify trends, data errors or issues, and gaps in care
  • Work with clinical and non-clinical staff, information technology, and management to develop and implement quality reports/dashboards and quality improvement protocols
  • Basic data analysis (preparation of graphs and tables) to communicate quality measure results/interim data on an on-going basis
  • Assist in the creation and implementation of new clinical workflows and best practices
  • Communicates effectively and professionally with physician offices, clinical facilities, and NFCH staff to assist in various outreaches and program activities.
  • Review charts and create alerts/triggers to highlight quality improvement performance opportunities.
  • Perform data collection, data entry, and quality performance monitoring activities as applicable.
  • Scan relevant components of the medical record to support reviews performed
  • Support and serve as back-up for other programs in the department

The successful candidate will possess the following minimum qualifications:

  • Current LPN or RN license for the state of Florida.
  • Minimum of 2 years’ in a manager/supervisor role
  • Minimum of 2 years’ work experience in a healthcare setting involving patients with complex chronic disease states preferred.
  • Must be highly motivated, result-oriented with strong skills in presenting, communicating, organizing, multi-tasking and time management skills
  • Strong organizational and interpersonal skills
  • Excellent customer service skills demonstrated by positive feedback from patients/team.
  • Ability to identify problems and recommend solutions
  • Ability to read, write and communicate effectively orally and in writing
  • Basic computer skills including previous work with an electronic health record (EMR)
