Job Description
. Outpatient: · Scheduling and maintaining daily schedule. · Physician initial order or referral. · Diagnosis for therapy services including date of onset; initial evaluations and all re-evaluation in accordance to Medicare and Insurance guidelines. · Plan of treatments with MD signature pertaining to claim period. · Progress notes and attendance records pertaining to the claim period to support total minutes of actual treatments and number of minutes for each modality · Clinical progress notes. · Perform videlfluroscopic swallowing studies. 2. Hospital: · Patient education sheet. · Multidisciplinary care plan. · Education for nursing staff and PCTs. · Updated forms and policy. · Perform standardized patient testing on each patient when appropriates. · Perform videofluoroscopic swallowing studies.
Job Qualification
· Master’s Degree in Speech Pathology, required. · Currently licensed in Speech Pathology in the State of New York. · Certificate of Clinical Competency from the American Speech-Language-Hearing Association, required. · Eligibility for certification in Speech Pathology by American Speech and Hearing Association.
