Job Description
Evaluates and treats patients with hand and/or elbow injuries using physical, chemical and mechanical means in accordance with written and signed prescription and/or referral of a state licensed physician or authorized practitioner. Maintains complete medical records for each patient.
Job Responsibility
1.Administers direct patient care in accordance with the written and signed prescription of the referring physician or other state authorized practitioner.
2.Develops and implements treatment plans to improve gross and fine motor coordination, movement and function of upper extremities.
3.Instructs patient and caregivers in daily living skills including, but not limited to: dressing, feeding and household activities in order to promote a more independent and self-sufficient level of functioning.
4.Facilitates the use of Durable Medical Equipment (DME) and instructs patient in use and protocols for dealing with malfunction of same; makes appropriate recommendations for follow-up care upon discharge.
5.Submits required documentation in accordance with the policies and procedures of the department.
6.Participates in quality improvement chart reviews and activities.
7.Observes patient tolerance and/or status during treatment and modifies treatment program, as needed.
8.Designs and implements a treatment plan that best meets the needs of the patient and works toward achieving established goals.
9.Attends family meetings/conferences and interacts with members of the inter-disciplinary team pertaining to the welfare and management of assigned patients.
10.Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions.
Job Qualification
1.Bachelor’s Degree in Physical Therapy or Occupational Therapy, required.
2.Current license as a Physical Therapist or Occupational Therapist in New York State, required.
3.Hand Therapist Certification within two (2) years of employment, required.
4.1-3 years of relevant experience, required.
