IV. RESPONSIBILITIES AND DUTIES

A) General

Supervise the prescribed daily routines of the individual’s in accordance with applicable standards and regulations. Ensure that appropriate written documentation is completed as scheduled.

Responds to crisis situations expediently and provides the appropriate input and support needed to resolve the situation.

B) Specific

1. Provides relevant information, as requested, concerning individuals’ values, interests, needs and progress.

2. .

3. Assists individuals with any transitions such as Emergency room visits or new job placement.

4. Demonstrates knowledge of assigned individuals’ goals, Goal Plans, Supplemental Procedures, Behavior Support Plans and Nursing Plans of Care and assures they are implemented as written; maintains data as required/specified in clients’ Individual Plans.

5. Assures individuals participate in all required/scheduled meetings, appointments and daily activities and all job interviews.

6. Attends meetings as scheduled or as directed by the Supported Employment Director or his/her designee.

7. Immediately reports incidents according to Agency policy for Incident Reporting and Investigation Procedures.

8. Attends and participates in all in-service training as scheduled.

9. Provides individuals with consistent supervision and interaction. This includes, but is not limited to, staying on the work site until the individual is officially relieved by his/her supervisor.

10. Perform other duties as directed by the Supported Employment Director or his/her designee.

Job Type: Full-time

Pay: From $17.00 per hour
