North Star Gems In-home Care is looking for (1) Part time Healthcare In-home Care Social Worker to join our growing team.

**THIS JOB IS A TEMPORARY COMMISON BASED WITH THE OPPORTUNITY TO BECOME A FULL TIME SALARIED EMPLOYEE.

Responsibilities

• Coordinating the care of patients with complex needs that require multiple services over time.
• Market and promote business services to various physicians, attorneys, rehabilitative centers, physical therapist, discharge coordinators, outpatient centers, nursing homes,. home health agencies and administrative staff to care for all patients of all ages, from infants to the elderly as well as their families.
• Assisting patients with understanding financial and emotional resources available to them
• Reviewing new admissions for any special needs or issues that need to be addressed.

Qualifications

• Bachelor's degree in social work, sociology, or psychology
• Minimum 3 years experience
