Perinatal Family Support Specialist - ( 81220 ) PFSC Support Associate

Job Type: Part time

We are currently seeking a Perinatal Family Support Specialist for Evanston Hospital, with some coverage at Highland Park (Womens) Hospital, as needed. This is a part-time position and is benefits eligible. This position is 24 hours per week, Monday, Wednesday and Friday; several weeks of on-call are required throughout the year with the possibility of return to hospital, if clinically indicated.

Our program supports in and out-patient high-risk pregnant and postpartum teens and patients, neonates, pediatric patients, and their families. Additionally, we provide support around adoption and surrogacy, as well as obstetrical and gynecological surgeries; perinatal loss comprises part of the work. This position supports improved patient outcomes, safety, appropriate and timely treatment, and discharge.

In this role, you will:

* Perform clinical psychosocial assessment and/or crisis intervention related to a patients emotional and cognitive status, current and/or historic mental health concerns, and perception of diagnosis of self and/or fetus/neonate, coping skills, support system, resource needs and barriers to care.

* Provide social-emotional and environmental support services, referring to community resources, as needed

* Provide resources and referrals to behavioral health services

* Perform triage duties related to patient referrals

* Consistently compose and enter accurate, concise, thorough and timely documentation in the electronic medical record system

Qualifications

Our ideal candidate will have:

* Masters level degree from accredited institution in social work, counseling, or other behavioral health science

* LCSW, LCPC, PsyD; LSW (actively working towards LCSW licensure) or LPC (actively working towards LCPC licensure)

* Two years clinical experience required; 3-5, preferred

* Two or more years of experience with high-risk, at-risk pregnant persons and families, preferred

* Knowledge and understanding of mental health risks during the perinatal period

* Knowledge of the developmental needs of the neonate to one month

* Knowledge of the needs and strengths of the adult and teen maternity patient, pediatric patients

* Knowledge / experience with perinatal loss

* Professional writing skills

* EMR/EPIC experience preferred

* Strong team participant

* Bi-lingual Spanish skills preferred, but not required
