NorthLakes Community Clinic is currently looking for a full-time Pharmacist to join our clinic location in Iron River, Wisconsin!
The purpose of a pharmacist is to achieve positive outcomes for patients through the use of medication to improve a patients' quality of life. A pharmacist is successful when they educate and advise providers, and other health professionals on medication therapy decisions, dispense medications and monitor patient health and progress to optimize their response to medication therapies with minimum risk.
Prepares medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities. Dispenses medications by compounding, packaging, and labeling pharmaceuticals. Controls medications by monitoring drug therapies; advising interventions.
  • Take in the prescription from the patient or via Electronic Health Record and follow the proper procedure in assessing the appropriateness of the medication in accordance to the patient's medical history.
  • Works with the pharmacy technician to prepare the correct amount of the prescribed medication.
  • Conduct final check of the prescription before the drug is given back to the patient.
  • Accurately maintain confidential records of all patients.
  • Accurately log and track medication inventory.
  • Report any discrepancies to the Pharmacy Director
  • Utilize exemplary customer relations in interactions with patients.
  • Contact and work with insurance companies to ensure best possible outcomes for the patients in terms of drug efficacy and cost.
  • Review relevant literature, synthesize evidence-based materials, and translate complex behavioral health concepts
  • Conduct case management and other administration tasks as needed
  • Collaborate with NorthLakes providers in the best interest of the patients' continuation of care and prescription therapies.
  • Work collaboratively with NorthLakes support staff in the interest of care integration initiatives.
  • Appropriate use of PPE/infection control protocols when providing care to patients.
  • Plans and administers therapeutic treatment by telephonic/ telehealth methodologies as indicated by patient needs and state guidelines.

Requirements
A current, valid, and unrestricted Pharmacist or Pharm D license to practice the profession in Wisconsin
Preferred Skills
To perform this job successfully, an individual should have knowledge of Electronic Health Record Database software, Word Processing software and Pioneer RX pharmacy software.
