We serve patients of all ages who have been diagnosed with serious chronic diseases like diabetes, hypertension, and childhood obesity. On any given day YOU could be meeting in the morning with a newly diagnosed patient to discuss the services that our team offers, making contact with a patient whose condition is getting worse by lunch, and teaching a child and their parents about healthy lifestyle changes that would greatly improve their wellness and reduce health risks in the future. Our MA-CHWs make a difference in their patients' lives everyday!

This understanding enables the CHW to serve as a liaison, link and educator between services and the community to promote services and ultimately enhance community health. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, data collection, social support and advocacy.
