Summary:

The Quality & Patient Safety Clinical Coordinator position is responsible for establishing, coordinating, and participating in the development of Performance Improvement (PI) Plans to ensure the highest level of quality, safety, and patient experience within the practice. The Coordinator will work at the direction of the Practice Manager and Lead Provider and in partnership with the Hospital Quality Department. In addition, s/he will require close collaboration with staff within the clinical departments, data analytics, financial and business planning groups throughout the Member Organization. S/he will analyze, develop, and implement workflows to achieve best practice to maintain compliance against standardized benchmarks and definitions under the Joint Commissions, Quality Oncology Practice Initiative (QOPI), American College of Surgeons Commission on Cancer Accreditation, Dana Farber Cancer Care Collaborative and the Institute for Safe Medication Practices (ISMP). Responsible for working in partnership with the interdisciplinary team to analyze patient experience outcomes and incorporate evidence-based research into clinical practice to improve patient experience. Stays abreast of pertinent regulatory and compliance issues and ensures adherence with all federal and state related laws and regulations. Protects all patient information and institution assets and complies with privacy and acceptable use policies.

Responsibilities:

  • Collaborates with the interdisciplinary team, Quality Committee, and nursing leaders to define key strategic priorities in the areas of patient safety and quality improvement.
  • Leads and supports patient safety and quality improvement initiatives within the Practice to ensure the safe, compliant, effective, and patient-centered quality of care.
  • Directs complex initiatives and responds quickly to changing issues and priorities involving multiple departments and stakeholders.
  • Reviews, refines, and collaborates to establish and maintain clinical standards based on National Patient Safety guidelines and standards of excellence in alignment with Northern Light Health quality, safety, and compliance initiatives.
  • Conducts data analysis to identify trends and patterns related to care outcomes.
  • Develops actions plans to address negative findings and provides dashboard to Practice Manager and Clinical Lead and Quality Committee outlining findings against National Standards and organization KPI’s.
  • Collaborates with leadership, IT and others to identify and implement key process changes required for efficient practice operations and adherence to best practice standards.
  • Works with clinical staff to implement streamlined and efficient workflow changes. • Attend Quality Committee, Medication Safety Committee, Staff, and Hospital Quality Committee meetings, and monthly Provider Meetings.
  • Responsible for facilitating any QOPI, or Dana Farber on-site visits.
  • Coordinates, integrates, and completes compliance reviews of new clinical policies and standard operating procedures.
  • Maintains open and effective communication with all providers, employees and vendors.
  • Partners with outside healthcare partners (Other Northern Light Health Member Organizations, NECS, Dana Farber, etc.) as needed to support practice initiatives and strategic objectives.
  • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, personal networking and participation in professional societies.
  • Meets with Leadership on a regular basis to keep them informed of performance against quality and patient safety performance metrics.
  • Collaborate with members of the multidisciplinary healthcare team to identify areas for improvement.
  • Provide training to teams to reduce quality problems and increase understanding of quality goals.
  • Monitor corrective actions to ensure results are capturing intent and maintaining sustainability.
  • Obtain and analyze reports from the electronic health record and quality data software.
  • Organizes, compiles, and reports QI data to identify trends, establish priorities, and recommend improvement activities.
  • Performs comprehensive, concurrent, and retrospective reviews of data in a timely manner.
  • Assists with staff education and training on quality improvement and risk management projects and tools.
  • Maintains accountability for ongoing self-development activities, especially related to, regulatory standards, presentation facilitation, change management and the application of quality improvement activities.
