Per diem telehealth physician or nurse practitioner

Northern Light Home Care and Hospice in Maine is seeking a per diem physician or nurse practitioner to offer home telehealth services to patients in our communities. This excellent opportunity would involve reviewing clinical data, collaborate with telehealth nurses, provide supervision and make appropriate alterations to the patient's care plans as needed. Majority of patients are home-bound, on palliative care or other serious illnesses, such as COPD and heart failure. Your participation assists in tremendous clinical outcomes by increasing patient satisfaction, reducing readmissions and optimizing clinician workflow.
