Northern Light Home Care and Hospice

Department: Hospice - South Portland

Position is located: Northern Light Home Care & Hospice So Portland

Work Type: Full Time

FTE: 1.0 (40 hours)

Work Schedule: 8:00 AM to 5:00 PM

This is for our Immigration to Work Program available to New Mainers

Summary :

The Patient Companion is responsible for providing assistance with Instrumental Activities (IADL’s) of Daily Living (examples: shopping, preparing meals, housekeeping, laundry, etc) to a designated group of patients in their homes as directed. The incumbent is responsible for para-professional aspects of care and patient safety. The Patient Companion collaborates with the department leadership, family and members of the interdisciplinary group. Performance reflects the mission and values of Home Care & Hospice.

Responsibilities:

  • Provides companionship, assistance with Instrumental Activities of Daily Living for patients as assigned and under the direction of Home Care & Hospice staff.
  • Recognizes symptoms and changes in the patient’s condition and provides feedback to the interdisciplinary team to assist in the evaluation of care.
  • Reports concerns about patients or family to the member of the interdisciplinary team who is coordinating homemaker services.
  • Provides service and produces work at or above expected levels of quality and consistent with agency defined qualitative standards.
  • Documents accurately, completely and in a timely manner.
  • Performs duties as assigned on the Homemaker Care Plan.
  • Participates in in-service and continuing education programs, staff meetings, and patient conferences as requested.
  • Other Duties as assigned.

Other Information :

Current license to drive in the state of Maine, with reliable transportation and agency required auto liability insurance. Must be able to travel within assigned region, or statewide, depending on the position.

Education
