Northern Light Maine Coast Hospital 

Department: Nutritional Counseling

Position is located: Ellsworth Family Practice

Work Type: Full Time

Hours Per Week: 40.00

Work Schedule: 7:00 AM to 4:00 PM

Summary:

Provides Medical Nutrition Therapy to outpatients and inpatients as referred by the primary care providers and hospitalists. Provides individualized diabetes self-management nutrition education to individuals and groups as the primary dietary instructor for the DSMT program. Provides nutrition assessments on inpatients according to policy.

Responsibilities:

  • Maintains patient confidentiality.
  • Delivers lectures with professionalism and accuracy.
  • Directs patients and families to appropriate community resources.
  • Acts as a patient advocate.
  • Maintains access for patients through telephonic, individual or group appointments as advertised.
  • Participates in the development of training materials.
  • Incorporates interactive education into DSMT program through cooking and grocery shopping experiences.
  • Participates in performance improvement activities to address opportunities for improvement.
  • Develops therapeutic diet materials as needed for nutrition education.
  • Meets with nutrition services to communicate inpatient dietary needs.
  • Participates on the DSMT Program Advisory/Oversight Committee.
  • Implements methods to improve communication with patients and family members.
  • Provides documentation of patient education and medical nutrition therapy in EMR for provider review.
  • Attends inpatient discharge planning/swing bed meetings as able.
  • Acts in an advising capacity to nutrition services regarding menus and therapeutic diets.
