Description

POSITION TITLE: YOUTH CASE MANAGER

REPORTS TO: Youth Program Director

FLSA CLASSIFICATION: Non-Exempt

Pay Range - $18 to $24

GENERAL RESPONSIBILITIES: Provide ongoing support, advocacy and guidance to participants in order to help them deal with their personal, legal, and social service needs, especially those that interfere with their ability to meet Program requirements and their personal goals. To coordinate and develop post-secondary placement, job opportunities, and customer support for participants and graduates of the Northern Kentucky Community Action Commission Youth Program, and to instill a sense of citizenship, community pride and personal responsibility within the Future Leaders that we serve.

SPECIFIC RESPONSIBILITIES:

  1. Work as part of the Youth team to enroll eligible participants (16-24 years of age) into the program, including eligibility determination and completion of required documentation, enrollment in the required databases, as appropriate and tracking of ongoing eligibility and participation – in line with agency and program Policies & Procedures.
  2. Counsel and support Youth Future Leader trainees to provide supportive services, job search, placement and retention. Work as a team in assessing personal and career needs of Youth participants and finding resources to meet their needs. Meet regularly with staff to develop and maintain a consistent approach to supporting and expanding the personal growth and leadership skills and mentoring of program members.
  3. Work with the local community to establish partnerships that can provide service opportunities and internships for Youth participants.
  4. Keep detailed case notes on services, outcomes, hours and certifications achieved. Keep detailed service records of contacts with, and issues relating to, participants; providing case management; reporting to funding and other authorities routinely as and when needed. Included in this is program services documentation, ongoing eligibility, and outcome/exit documentation.
  5. Complete initial and ongoing Individual Success Plans for all participations, according to program guidelines.
  6. Coordinate job training opportunities with other staff and community partners.
  7. Collaborate as needed with community partners on participant training, education, internship placement, and job placement.
  8. Make home visits, as appropriate, to assess needs and to support participants.
  9. Ensure completion of semi-annual performance evaluations. Supply primary input for evaluations to provide reports on trainees’ and graduates’ job readiness, individual development, job fit, and retention.
  10. Identify and share post-secondary education and employment opportunities; and direct trainees and graduates to such opportunities, assisting with application, placement, monitoring, assistance, and oversight of program trainees and graduates in such situations. Collaborate with other employment training and support programs.
  11. Prepare weekly, monthly, quarterly & annual reports required by funders & the NKCAC.
  1. Maintain and develop strong professional relationships with private and public sector employers. Identify and match skills and needs of trainees and graduates to employer requirements. Advocate for participants and graduates to local employers. Provide on-going retention support, job coaching, and assistance to placed trainees and graduates.
  2. Create and maintain written & electronic records of contacts, relationships, and outcomes with placed trainees and graduates and employers, demographic information, job search tracking, job retention tracking, pre-employment processing of trainee timesheets.
  3. Research and analyze industry norms and market economic developments and provide recommendations to improve programs’ activities.
  4. Design and administer leadership development activities as defined by the Program Director. Organize and conduct employment readiness seminars for participants on a weekly basis.
  5. Assure initial and on-going compliance with all program and grant requirements
  6. Conduct other duties as assigned.

Qualifications


QUALIFICATIONS:

  • Bachelor’s degree in social service, counseling, psychology or related field; or equivalent experience and training preferred.
  • Minimum of three to five years of experience in social services organization.
  • Supervisory or program management experience preferred.
  • Proficiency in Microsoft Office applications and on-line databases is required.
  • Committed to helping young people succeed in an innovative training program to re-orient their lives, nurture their leadership skills and enable them to make a difference in their community.
  • Prior training and/or experience working with disadvantaged and at-risk youth preferred. The ability to establish rapport and relate sensitively to a low income, multiracial and multicultural group of young people.
  • Knowledge of human service, health, court and criminal justice systems’ programs, regulations and procedures and alternative programs.
  • Excellent speaking, listening and written communication skills.
  • The ability to work as part of a team.
  • Must have valid driver’s license and reliable transportation.
  • Must be able to work with a diverse workforce.

PHYSICAL REQUIREMENTS:

  • Ability to lift and carry up to 50 lbs 25% of the time.
  • Ability to sit at a computer for up to 8 hours a per day.
