A Case Facilitator II – Adult operates under supervision of the assigned Area Director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include client reassessment and completion of ANSA, appropriate assignment of cases for direct service, monitor of client treatment episodes, recovery plans and reviews, referral and linkage to services, and client terminations
First Shift, full time.
Education: Bachelor’s Degree related to mental health.
Experience: Two years' experience in mental health or community services preferred = Must meet OBHP requirements.
Licenses: A valid state issued driver's license.
Salary based on experience.
