Position: Medical Planning Specialist - (LPN) CUA 1

Hours: Varies depending on Caseload

Location: 2701 N. Broad Street Philadelphia, PA 19132

CUA 1 (25th Police District) Feltonville, Hunting Park, Upper Kensington, McGuire, Fairhill, Hartranft and Franklinville

Summary of Position:

The Licensed Practical Nurse (LPN) performs nursing functions to support and enhance the Community Umbrella Agencys (CUA) capacity to address the needs of children with medical complexities and or children who are medically vulnerable. This is a support that is offered in the home of children receiving In-Home services and supports the needs of all children CUA-wide as it relates to serving planning consultation, tracking of medical records, assessing and monitoring progress of children with medical conditions.

ESSENTIAL POSITION RESPONSIBILITIES: (and other duties as assigned)

  • Visiting the homes of children with medical complexities or who are medically vulnerable for the purposes of observation, education, safety planning, progress monitoring and intervention. Visitation will be based on case-management safety plan and/or single case plan.
  • The collection, maintaining and synthesizing of information from a childs medical records and ensuring that actions steps are put in place to address any issues noted from received medical records.
  • Scheduling appointments as needed for children on caseload and help parents create appointment calendars.
  • Ensure that parents of newborn children understand the periodicity schedule for infants and ensure infants and parents adhering to appointment schedule.
  • Attend medical appointments as needed of requested.
  • Mandatory DHS nurse consultations for all medically vulnerable children.
  • Attend single case plan meetings as requested by case-management chain of command for children on caseload and are medically vulnerable or if it is unclear if the child is medically vulnerable.
  • Educate parents, age-appropriate children and case managers as it relates to managing and understanding a medically vulnerable childs health interventions and the consequences thereof.
  • Provide nursing assessment of clients requiring medical stabilization due to hospital discharge for any reason.
  • Assure that children on caseload are in accord with all medications and medical treatments as ordered by a childs PCP or discharge documentation from hospital.
  • Verifies case childs prescriptions and usage as ordered by childs physician, to provide for continuity of care.

Qualifications


Minimum Qualification:

  • Graduate of an accredited nursing program
  • Current nursing license, in good standing with the State licensing board
  • Minimum of one (1) Pediatric or Community nursing experience preferred
  • Adhere to applicable city, county, state, federal laws, regulations, NET, NET Community Care, policies, and Department of Human Services (DHS) city and state, CBH licensure and CARF standards.
  • Cognitive skills as related to the position
  • Valid Drivers License
  • Current CPR/First Aid/AED certification
  • Current PA Child Abuse Clearance
  • Current FBI Clearance
  • Current PA Criminal Clearance
  • Health Assessment and TB Test required
  • Ability to work with diverse populations
