The Blended Case Manager works with the serious and persistently mentally ill population, using a strength based perspective, to assist clients to obtain necessary services and support. The primary goal is to empower the client to identify their needs and choose the resources to fulfill them. This workforce member has access to the complete chart for cases requiring his/her workers involvement.

Bachelor's Degree in social work, psychology, or related field is necessary. Must maintain a valid PA Driver's license.
