Northeast Center for Rehabilitation and Brain... is Hiring a Social Worker- Discharge Planner Near Lake, NY

Northeast Center for Rehabilitation and Brain Injury is a one-of-a-kind rehabilitation and community re-entry facility in Lake Katrine, NY (near Kingston, NY), specializing in:

  • Neurobehavioral Disorders
  • Complex Medical Recovery
We are the largest brain injury treatment program in the world. We are a facility where talented people are given the opportunity to use their special skills, be creative and sometimes push the envelope.
Responsibilities:
  • Assessing psychosocial needs, adjustment to illness and community re-entry barriers by completing comprehensive discharge intake.
  • Participate in comprehensive care plans meeting with interdisciplinary teams, resident neighbors, families / designated representative.
  • Explore discharge planning/community options, including Medicaid TBI waiver, NHTD waiver, OPWDD waivers and lateral discharges Maintain communication with Open Door, Front Door programs when assisting with community transition. Interface within house OASIS program to discuss barriers to discharge.
  • Complete entitlements/benefits, complete housing forms.
  • Compile discharge packet/summary to include, but not limited to community medical appointments, accessing CHHA/nursing services, arranging transportation, working with Waiver services/coordinators, completing Notice of Discharge.
  • Maintain open lines of communication with designated representatives/legal guardians.
  • Communicate with all disciplines in building strong alliances and work groups.
  • Participate in Performance Improvement activities.
  • Enhance professional growth through participation in educational services and community involvement.
  • Master Level Supervision, hours can be flexed, availability to work weekends.
Requirements:
  • Bachelor of Social Work (BSW) degree, or Bachelor degree in a related field with one year experience in social work/discharge planning or case management.
  • Communicates in a clinical and professional manner.
  • Experience in office technologies, computers, word processing and software programs. Has good organizational skills. The ability to communicate well with department staff. Has a pleasant and professional telephone persona.
  • Must be able to read, write, speak and understand the English language.
  • Possess the ability to make independent decisions when circumstances warrant.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle personnel based on whatever maturity level at which they are currently functioning.
  • Must possess leadership ability and the willingness to work harmoniously with and supervise other personnel.
  • Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures of the facility.
  • Must maintain the care and use of administrative supplies, equipment, etc., and maintain the work area in a clean, safe and attractive manner.
  • Must be knowledgeable of computers, system applications, and other office equipment.
