Registered Nurse-Full Time, Part Time, and/or Per Diem

Northeast Center for Rehabilitation and Brain Injury is a one-of-a-kind rehabilitation and community re-entry facility in Lake Katrine, NY (near Kingston, NY), specializing in:

Northeast Center for Rehabilitation and Brain Injury has just opened up The Premier Unit, a new, state of the art, 40-bed short term rehabilitation unit and we are looking to hire 5 Registered Nurses (RN) to the team.

Responsibilities:

  • Assess, develop and implement adaptable care plans, and evaluate and modify care plans to achieve goals and objective
  • Develop goals for clients, families, and rehabilitation teams, participates in conferences and meetings, and works with rehab team members.
  • Direct the clinical operations of a 40-bed short term unit with a team of well-trained LPNs and CNAs.
  • Collaborate with a strong Medical, Social Services, Nutritional Services, Rehabilitation and Recreation Departments.
  • Provide leadership to the interdisciplinary team to provide highest quality of care and services to residents of the facility.
  • Notify the resident’s attending physician, as well as the resident’s legal guardian and/or designated representative with any changes in condition or other clinical events.
  • Collaborate with the RN Wound Nurse, RN Infection Preventionist, ADON and DON as needed.
  • Ensures written policies and procedures are followed and nursing staff are following their job duties
  • Renders professional nursing care to neighbors on assigned unit
  • Performs nursing techniques for the comfort and well-being of the neighbors.
  • Assesses neighbors throughout shift to ensure all needs are being met
  • Administers prescribed medications and treatments and observes outcomes, changes dressings, observes and documents on wounds, performs and documents vital signs
  • Completes weekly, daily nursing notes, updates Care Plans as needed
  • Completes Event reports before the end of shift
  • Ensures rooms are ready for new admits, greets new admits and families, and escorts to room
  • Makes rounds with physician
  • Notify physician and next of kin of any changes in condition
  • Give/receive report coming on and going off duty
  • Performs narcotic count at the beginning and end of shift, looking at both the book and the medication to ensure accuracy
  • Administer professional services such as but not limited to: catheterizations, GT changes and care, trach care and suctioning, care of ventilated neighbors, peripheral IV insertion and maintenance, responds to Code Blue, administration of IV, PO, IM, SC, rectal, vaginal medications, enteral feeding via GT, JT, G-J tubes suctioning, ostomy care, dressing, wound vacs, central line care and dressing changes, obtaining lab specimens, vital signs, head to toe and focused assessments and documenting such
  • Performs a total physical assessment of all injured/ill neighbors.
  • Follows and ensures unit staff follow infection control policies
  • Attends in-services
  • Recognizes unsafe and unclean conditions and implements corrective action to provide neighbors and personnel with healthy, clean and safe environment.
  • Deals effectively, professionally and tactfully with neighbors and family complaints.
  • Observes for undesirable visitors within the facility and assists in maintaining internal and external security.
  • Maintains staff morale and promotes their working together as a unified team.
  • Assists in orientation and on the job training of new employees and ongoing education of staff
  • Performs direct neighbor care and other job related duties as required.

Requirements:

  • Current NYS Registered Nurse (RN) licensure
  • Will consider graduate nurse (GPN)
  • Short Term/Sub-Acute care experience preferred
  • Must be able to read, write, speak, and understand English.
  • Must have demonstrated ability to make decisions with good nursing judgment, handle emergencies and maintain excellent patient assessment skill.
