Primary Purpose of Position: Under the direction of the Manager of Patient Services, the Referral Coordinator will ensure proper processing of incoming specialist referrals for specialty care appointments/testing. The Referral Coordinator will also track referrals and process them in a complete and timely manner. Essential Functions: • Process all appropriate referrals/ prior authorizations that are needed • Provide appropriate clinical information to specialist • Ensure accurate patients demographic and current insurance information • Maintain appropriate documentation on the referrals to promote team awareness and efficiency with the referral tracking • Weekly referral tracking on all specialist appointment using DRVS • Requesting consult notes • Rescheduling appointments when necessary • Communicate professionally with all individuals associated with the referral process both internally and externally • Collaborate with Nursing, Providers, Support Staff and Outreach and Enrollment to ensure quality referrals • Respect and maintain patient confidentiality Non-Essential Functions: • Attends periodic staff meetings • Adheres to NSCH policies • Adheres to NSCH values and principles: Mission North Shore Community Health’s mission is to build healthy communities by providing exceptional care to all. Requirements

Requirements and Qualifications: • High School Diploma • Must have one year of experience in a health care setting • Understanding of Medicare, Medicaid and managed health care • Ability to work in a culturally diverse environment • Team work Orientation • Strong organizational and communication skills • Ability to be detail oriented • Good problem-solving skills • English/Spanish bilingual ability preferred • Effective November 1, 2021, North Shore Community Health (NSCH) will require all its employees to be vaccinated against COVID-19, as a condition of employment, unless a valid religious or medical exception is granted. All prospective employees will be required to be fully vaccinated before their first day of employment. As of 9/1/2021 fully vaccinated means two doses of Pfizer or Moderna, or 1 dose of Johnson and Johnson at least 14 days prior to your start date, or in the event of an immunocompromised person, three doses of Pfizer or Moderna 14 days prior to start date. Proof of full vaccination status must be provided as part of their mandatory pre-hire paperwork. As a health center, NSCH ensures that all employees comply with the annual requirements for vaccinations and health screenings.
