Program Overview : Integrated Care

Integrated Care - North Range recognizes that wellness is based on the interconnection between physical, mental, emotional, and spiritual health and approaches individuals with each of these components in mind.

Integrated care allows for better continuity of care through improved communication and care coordination between primary care and behavioral health providers.

Behavioral health services include assessments, therapy, crisis management, and care coordination.

Shift :

• Monday, Tuesday, and Friday : 9 : 00 am 1 : 00 pm
• Wednesday and Thursday : 1 : 00 pm 5 : 00 pm

Salary Range :

Salary Placements will vary and may be above the starting wage based on factors including but not limited to location, experience, education, and licensure.

Employees may be eligible for additional pay based on program, evening, night, weekend, or holiday work. Bilingual pay may be available upon successful passing of language validation testing.
