Job Description

**_Patient Care:_** Provide nursing care to facilitate high quality patient care; scheduling patients; calling in medication refills; administering medications; working up patients; providing skilled nursing procedures; screening lab/test results; assisting physician/nurse practitioner with patients. **_Case Management:_** Provide case management to patients seen at North MS Medical Clinics. **_Liaison_** Act as a liaison between physician, patient, families and other office staff. Actively promote a patient-focused approach to care delivery. **_Triage:_** Triage walk-in patients and telephone calls. **_Documentation:_** Responsible for maintaining written or computer record of patient care events performed by the incumbent (e.g.: medications, treatments, procedures). **_Education_** Initiate, perform, and develop education materials. Provide and document patient teaching.
