General Description:

The Program/Project Coordinator oversees the day-to-day operation of the Well Woman HealthCheck Program (WWHP) in Coconino county. A critical role of this position is to oversee case management of clients in this service area. This position supervises two Patient Navigators stationed in remote sites. The Program/Project Coordinator identifies and mitigates screening barriers clients may encounter and assists them in completing their screenings, provides case management of needed diagnostics and initialization of treatment with the Patient Navigators. The Program/Project Coordinator must ensure Quality Core Indicators and screening goals are met and/or exceeded. This role educates providers and support staff about the program's covered services and referral mechanisms and facilitates culturally proficient education and support to patients eligible for cancer screening. This individual develops agreements with appropriate service providers for required services, obtains medical results and pays for approved services in a timely manner. The Program/Project Coordinator participates in activities focused on cancer screening and collaborates with community organizations and develops systems for increasing screening across the clinics. The Program/Project Coordinator will be supervised by the Program/Project Manager.

Essential Duties:

  • Budget, grant, and staff management ensuring quality/timely reporting and utilization
  • Maintain client list, case management tracking forms and develop monthly client expenditure reports.
  • In conjunction with the Program/Project Manager, develop PR strategies to publicize the program and special events to ensure utilization meets/exceeds annual goals.
  • Ensures team is recruiting potential new and annual cancer screening participants from priority populations through referrals, mailings, phone, and/or face-to-face contact which may be either in large group and/or one-on-one environments. Plans screening days in collaboration with team.
  • With Program/Project Manager, identifies and secures community partners and contracted service providers are in places and adhere to standards for timely and quality care.
  • Identify/convene collaboration with community organizations to reach health disparity population.
  • Disseminate information regarding other services provided by our organization as well as external agencies; promote staff referrals to social service agencies that may improve quality of life (such as to secure medical insurance, nutrition, and shelter).
  • Assist with cancer screening and diagnostic education of clients and clinic patients, as needed.
  • Verify timely completion and return of enrollment paperwork and assist clients with completing paperwork as needed.
  • Track clients, contacts, keep case notes, appointments and screening & diagnostic outcomes in appropriate spreadsheet and/or database and follow-up with clients as needed.

Skills/Position Requirements:

  • Supervisory experience
  • High level of Customer Service training or work experience with confidential information (medical, HR, legal, financial, etc.).
  • Strong computer skills (efficient with Microsoft Office, medical practice management software, databases, budget and labor tracking software).
  • Professional verbal and written communication skills a must.
  • Travel- for meetings, site visits, education/training (evening, weekends as needed).

Preferences:

  • Bachelor degree in Health Related Field or Equivalent Experience.
  • Bilingual, Spanish/English speaking.
