POSITION SUMMARY:

The Transportation Coordinator reports to the Population Health Program Manager. The Transportation Coordinator will work as part of a team to provide specialized transportation services to patients of the Health Center who face challenges accessing transportation options in order to receive health services. This position will function as the liaison for all non-emergency patient transportation needs. Will work directly with the Population Health Program Team to efficiently schedule and monitor driver logistics and communication with other coordinators, and patients. Will work directly with other community transportation providers. Also, prepare reports or recommendations on transportation planning. Plan, develop, or implement safety and security activities related to patient transportation. Coordinate with the Health Center’s Facility Manager to inspect physical conditions of vehicle fleets, or equipment and order testing, maintenance, repairs, or replacements. 

QUALIFICATIONS:

• High School Diploma or GED equivalent – some college preferred. Minimum one to three years’ equivalent job experience required
• Valid and acceptable NYS Driver’s License
• Pass a NYS Criminal History background check, Statewide Central Registry check and NYS Sex Offender Registry check
• Ability to work with various computer applications
• Organization
• Proficient with time management
• Excellent communication skills, and follow-through
• Strong problem solving skills
