JOB TITLE: Community Health Worker
LOCATION: Petoskey, MI 49770
REPORTS TO: Behavioral Health Home Coordinator
STATUS: Full-Time, Non-Union, Non-Exempt
HYBRID ELIGIBLE: Community Based position
STARTING WAGE RANGE: $16.95 - $17.79 per hour
QUALIFICATIONS:Education: Minimum of a High School Diploma. Certification as a Community Health Worker (Provided by NCCMH), Certified Nursing Assistant (CNA), Medical Assistant (MA), or LPN preferred.
Experience: Two or more years of experience working with persons with severe and persistent mental illness preferred. Lived experiences with persons with intellectual and developmental disabilities, and/or substance use disorders valued.
Other: Ability to obtain CHW certification. Ability to maintain the competency level as defined within the job classification and specific clinical practice. Ability to communicate professionally, verbally and in writing. Ability to work independently and as a team are essential. Knowledge of medical terminology preferred.
SUMMARY OF RESPONSIBILITIES: This position acts as a liaison or link between the client, health services, social services, and the community to facilitate access and improve health outcomes by increasing health knowledge for beneficiaries; this will be accomplished through a range of activities including outreach, community education, information and resource provision, social support, and advocacy. This position is responsible for supporting those we serve in recovery, maintaining good mental and physical health, and managing symptoms in support of healthy goals.
ESSENTIAL JOB FUNCTIONS:
  • Demonstrate universal precautions, standards or practices including but not limited to confidentiality, recipient rights, emergency protocol and follows HIPAA standards in safeguarding client information.
  • Assist in identifying and engaging clients who would benefit from CHW support. Provide input on development plan of care.
  • Recognize and identify needs of beneficiaries to ensure adequate internal and external resources are provided for health, safety, and social determinants of health.
  • Identify, contact, and establish ongoing relationships with community stakeholders and providers to support medical and psychosocial needs of beneficiaries.
  • Effectively provide BHH communication, written and oral, for community stakeholders and providers specific to the service they offer.
  • Create and maintain community resource guide for beneficiaries to utilize and maximize wellness throughout NCCMH catchment area.
  • Meet regularly with BHH team to plan care and discuss cases, exchange appropriate information with the team members in an informal manner, including participation in client care huddles with appropriate community providers and NCCMH staff.
  • Coordinate and communicate with medical providers, subspecialty providers including mental health and substance use disorder service providers, long term care providers and hospitals.
  • Assist with obtaining follow up care, referrals, and conducting referral tracking.
  • Monitor the effectiveness of interventions in care plans and provide feedback to care plan author. Provide education regarding health conditions and strategies to implement care plan goals.
  • Provide home visits as needed to ensure stability through transitions.
  • Advocate for individual and family needs.
  • Use of Electronic Health Record (EHR) and other Health Information Technology (HIT) to link services and facilitate communication among team members and provide feedback. Ensure that documentation is accurate and completed in a timely manner.
PHYSICAL REQUIREMENTS:Ability to walk, bend, stand, sit, lift to 50 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive a vehicle. Position requires physical ability to complete and maintain certification in CPI techniques which will be a training class provided to the job incumbent. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions in accordance with applicable laws.
OTHER REQUIREMENTS:
  • Must be current in Adult CPR and First Aid; provided by NCCMH.
  • Complete initial training in Non-Violent Crisis Intervention (CPI); provided by NCCMH.
  • Have own phone for multi-factoring, communication, and reliable access to messages.
  • Have reliable transportation in carrying out agency duties.
  • Possess a valid Michigan’s driver’s license and have a satisfactory driving record.
  • Maintain confidentiality with regard to all consumer data.
  • Must have strong interpersonal and communication skills and the ability to work effectively as part of a team with a wide range of constituencies in a diverse community.
  • Working knowledge of physical health, chronic health conditions, health education theory and practice, and behavioral health medications.
  • Competent in basic computer and typing skills; familiar with office practices, procedures and policies.
  • Ability to work in an autonomous role, to organize, prioritize, and schedule work assignments.
  • Ability to interact with physicians and other healthcare providers in a professional manner.
  • Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies.
  • Supports a philosophy of service delivery that is recovery-based, person centered and culturally competent.
  • Adhere to applicable policies and procedures as required by the Department of Community Mental Health, the Mental Health Services Board, and other pertinent regulatory agencies.
