- Registered Nurse or LVN

Key Responsibilities:
- Coordinate and complete the MDS assessment for all residents in a skilled nursing facility
- Work closely with the interdisciplinary team to ensure accurate and timely completion of the MDS assessment and care planning process
- Ensure compliance with all state and federal regulations related to the MDS assessment process
- Provide education and training to staff on the MDS assessment process
- Participate in quality improvement initiatives related to the MDS assessment process
- RN or LVN
- 3 years Minimum Data Set experience
- Skilled Nursing Facility experience
- Strong attention to detail
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
