JOB SUMMARY:

The Community Health Nurse will provide valuable nursing services in the homes and schools of the Nooksack Tribal Community with emphasis on a family-centered preventative approach in accordance with established policies and procedures. Identify medically related needs in the community and take part in outreach as well as one-on-one home visits with patients. The CHN will be responsible for developing and maintaining home health programs, grants and community events.

MAJOR TASKS AND RESPONSIBILITIES:

  • Develops and maintains Community Health Programs, and grants.
  • Oversee Community Health Representative program
  • Assists clinic providers by securing and maintaining a health and developmental history from the patient, parent, or guardian.
  • Assess and evaluates patient’s status by: 1. Writing and Initiating Plan of Care 2. Providing professional nursing care by utilizing all elements of the nursing process 3. Assess & Evaluates patient’s status by: re-evaluating and revising the Plan of Care as needed.
  • Provides general nursing care to patients in clinic, home or school setting.
  • Provides direct nursing care under physician supervision to patients with a range of health problems. Follows/interprets provider’s orders and communicates changes & assessments appropriately.
  • Case Management of chronically ill patients, with coordination of local health resources.
  • Keeps accurate, timely, and complete records of patient’s progress and conditions.
  • Performs services within the limits of preparation and experience.
  • Establishes and maintains communication with Clinic and individual patient providers regarding patients seen, and refers to resources when deemed necessary.
  • Makes home visits and patient assessments at the request of the Clinic Providers and schedules weekly patient load appropriately.
  • Coordinates with other community based agencies providing in-home care.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts for self-sufficiency and independence.
  • Provides appropriate post-discharge follow up care including dressing changes, assessment of vital signs, medication management, anticoagulation monitoring, and other in-home treatment.
  • Assesses, monitors, and supervises diabetic patients including foot care, home glucose monitoring, diet and activity, and insulin self-administration. Assists in coordinating monthly Diabetes Clinics and attend monthly diabetes meetings.
  • Attends Tribal Community Events to conduct public health education.
  • Oversees patient medisets by: A) Delivering medication on a bi-monthly basis & as needed. B) Monitoring medication compliance. C) Assessing for necessary medication changes.
  • Provides prenatal, newborn, and well-baby care by: A) providing home visits for environmental assessment and risk identification for prenatal clients, B) providing postpartum home visits after hospital discharge, C) Providing home and mothering assessment of all high risk infants, D) coordinating services for pregnant adolescents and provides home visits.
  • Coordinates care and advocates for elder population – A) provides home visits to monitor chronic illnesses and follow-up to hospital care. B) Cooperates with home care services in coordination of care for clients requiring personal medical home care. C) attends Elder’s Task Force monthly meetings. D) communicates with provider for various elder’s needs. E) Contacting the hospital discharge planner’s and the emergency room staff to determine need for follow-up.
  • Assists in Clinic when available, provides nursing support as needed.
  • Provides surveillance and adherence to medical regimens for stabilized chronically ill persons. This includes home visits as necessary, including assist with medication management and lab draws as ordered by patient’s providers, etc.
  • Provides healthcare and preventive services to healthy individuals, including guidance in nutrition, common illnesses, accidents, child growth and development and child rearing.
  • Provides teaching regarding the prevention and spread of various communicable diseases. Part of the instruction includes: A) Follow-up to clients with known or suspected communicable diseases. B) Immunization for all age groups and C) Monitoring the immunization status of the Nooksack Service population and coordinating care effectively with nursing staff.
  • Maintains contact and communication with the local health department and the community hospital(s) for the purposes of discharge planning and outpatient follow-up.
  • Coordinates Community Health Events.

OTHER DUTIES:

Because of the Tribe's commitment to community service and the well-being of its members, each employee may be expected to perform a wide range of office and field duties from time to time. Such duties may or may not be related to their regular responsibilities.

Indian Preference Policy applies to this and all positions with the Nooksack Indian Tribe.

MINIMUM QUALIFICATIONS

The following qualifications are required for the incumbent to have, in order to be considered for the position.

REQUIRED EDUCATION, EXPERIENCE AND TRAINING FOR POSITION:

  • Current, unrestricted WA State RN License
  • BSN
  • Minimum of 4 years nursing experience, home or community health experience preferred.

REQUIRED SKILLS/KNOWLEDGE/ABILITIES POSITION:

  • Must possess the ability to work independently without supervision; must be a self-starter and use good judgment in difficult situations. Must possess the ability to work in an ever-changing environment, remaining focused and adapt with ease.
  • Ability to independently plan, manage work, and coordinate programs
  • Ability to consult and negotiate with co-workers, private sector provider as well as Indian Community Members
  • Must be computer literate with specific working knowledge in Microsoft Office.
  • Must be able to communicate clearly, effectively and courteously with Nooksack Health Department staff, supervisor and clients, verbally and in writing.
  • Must continuously maintain a courteous and professional rapport with the clients and staff of the Nooksack Indian Tribal Community
  • Must be respectful and sensitive to the values and culture of the Nooksack Indian Tribal Community.
  • Must become familiar with the Nooksack Indian Tribal Community Health Departments and available resources available for patients, clients and their families.
  • Must have the ability to apply common sense and critical thinking skills when carrying out written and oral instructions
  • Must have the ability to effectively deal with a variety of interruptions that may arise during the course of performing employment essential duties.
  • Must be caring, empathetic and dependable.

    REQUIRED CONDITIONS OF EMPLOYMENT:

    • Must pass alcohol/drug test at time of hire and throughout employment.
    • Must pass criminal background check at time of hire and periodically thereafter.
    • Must adhere to Federal Privacy Act and comply with the Federal HIPPA regulations
    • Must provide record of immunization or positive Titer for MMR Varicella, 1-TDAP, Annual Flu shots and record of Hepatitis B series prior to beginning employment.

      REQUIRED LICENSES OR CERTIFICATIONS:

      • Must have and maintain throughout employment a valid Washington State Driver’s License and meet the insurability requirements of the Tribe. (Must Provide Driving Abstract at time of hire).
      • Current Basic Cardiac Life Support (CPR) and Emergency First Aid Certification (must be obtained within six months of hire and kept current thereafter).

        PHYSICAL REQUIREMENTS ( The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.):

        • Occasional standing/walking while performing office duties: walking short distances performed throughout shift on tile, carpet or concrete.
        • Frequent/constant sitting: on adjustable chair at desk or assigned work area. May stand as desired.
        • Ability to occasionally lift 1-20 lbs.: files, paperwork, supplies. Seldom required to lift up to 35 lbs.
        • Occasional push/pull using both arms/hands with required force of 5-20 lbs. to open doors/file drawers and filing document.
        • Seldom required to climb: stairs at some locations
        • Frequently required to bend at waist/knees/neck to and from seated position while working at desk, filing and records handling.
        • Not unusually required to kneel, crouch or crawl: may kneel or crouch occasionally to retrieve items from floor level.
        • Constant use of both hands/fingers and arms to reach/handle or grasp while keyboarding, using phone, calculator, copier, and other office equipment; writing instructions, filing and performing other administrative duties. Job requirements include forward and over shoulder reaching on a frequent basis.
        • Keyboarding – frequently for short intervals throughout work shift depending on specific work requirements.
        • Ability to use computers, reading fine print, interacting with the public and co-workers; visual requirements include hand/eye coordination and visual acuity in near/mid ranges.
        • Ability to constantly answer phones, communicate with clients, patients and, co-workers while giving and receiving instructions.
        • Work is performed indoors in a climate controlled office environment.
