Job Description:

We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to families in Coweta and Lagrange counties, helping them navigate through various challenges and connect them with the necessary resources. Responsibilities:

- Conduct comprehensive assessments of clients' needs and develop individualized care plans

- Provide counseling and support to clients, addressing their emotional, social, and behavioral needs

- Collaborate with community organizations and agencies to coordinate services and referrals

- Assist clients in accessing workforce development programs and employment opportunities

- Advocate for clients' rights and ensure they receive appropriate benefits, such as Section 8 or Medicare

- Offer crisis intervention and management for clients experiencing emergencies or difficult situations

- Provide addiction counseling and connect clients with appropriate treatment programs

- Offer mentoring and guidance to individuals involved in the child welfare system

- Coordinate childcare services for clients who require assistance

- Monitor client progress and maintain accurate records of services provided

Experience:

- Bachelor's degree in social work or related field (Master's degree preferred)

- Proven experience in case management or related field

- Knowledge of behavioral health principles and practices

- Familiarity with child welfare policies and procedures

- Strong communication and interpersonal skills

- Ability to work effectively in a team environment

- Excellent organizational and time management skills

Job Type: Full-time

Pay: $33.50 per hour

Expected hours: 40 per week
