Nimel Mental Health is Hiring a DIRECT CARE WORKER (PRP/CASE MANAGER) Near Westminster, MD

Nimel Mental Health is a fast growing and an accredited Outpatient Mental Health Center in Prince Georges and Carroll County. We are currently seeking direct care staff to provide service for consumers at our Carroll County/Baltimore region. The direct care staff will work in the community, home, or office-based settings with individuals experiencing mental/behavioral health challenges. Services provided are individualized and based upon a detailed client needs assessment.

Under the supervision of the rehabilitation specialist,the direct care staff will:

  • Complete assessment on clients' social, emotional, financial, health, or other problems to develop a rehabilitation treatment plan
  • Complete rehabilitation assessments identifying consumers’ strengths, rehabilitation needs, expectations of treatment, and preferences;
  • Develop and implement a individualized treatment plan with the client
  • Help clients resolve problems, identify resources and make effective use of resources
  • Organize and conduct psycho-educational groups
  • Develop and maintain relationships with other agencies, staff and local service providers to promote coordination and integration of care
  • Teach clients independence and self-advocacy
  • Be a vital part of the treatment team, collaborating with other agencies and individuals working with assigned consumers
  • Promote recovery and resiliency by providing rehabilitation services designed to meet the individualized needs of each consumer;
  • Meet with assigned consumers weekly, or as necessary, based on the individual needs of each consumer and agency policy;
  • Perform rehabilitation activities in the office, in consumers’ homes, and/or in the community;
  • Develop client specific, culturally appropriate intervention and strategies including goal setting, planning, and decision making;
  • Continually assess consumer progress, rehabilitation needs, appropriateness of services, and availability of formal and informal supports;
  • Complete and submit thorough, accurate, and timely documentation in accordance with agency policy and all applicable local, state, and federal regulations;
  • Participate in PRP trainings, supervision, and treatment team meetings;
  • All other duties necessary to fulfill program operation

Requirements:

  • Associate's/Bachelor's degree in a health-related field or college credit towards a bachelor degree in a related field
  • 2 years of work experience in a Mental Health setting
  • Must successfully pass all background screening required by the state
  • Completed CPR/First Aid training
  • Possess a valid driver’s license in good standing
  • Reliable transportation and auto insurance that complies with the minimum state requirements

Job Type: Part-time

Pay: $20.00 - $25.00 per hour

Experience:

  • Mental health counseling: 1 year (Preferred)
  • Case management: 1 year (Preferred)
