Nightingale Hospice is Hiring a RN Case Manager / Riverside Near Riverside, CA

As a RN Case Manger you will be planning, coordinating, and providing hospice care in accordance with the patient's plan of care. Working collaboratively with the patient, their family, and the interdisciplinary team.

Assessment and Evaluation

  • Conduct a comprehensive assessment of the patient's physical, emotional, social, and spiritual needs.
  • Evaluate the patient's medical history, current condition, prognosis, and any existing advance directives.

Developing a Plan of Care:

  • Collaborate with the patient, their family, and other members of the interdisciplinary team (e.g., nurses, physicians, social workers, spiritual counselors) to develop an individualized plan of care.
  • Identify the patient's goals, preferences, and priorities for care.
  • Address symptom management, pain control, emotional support, and spiritual care.

Coordinating Services:

  • Coordinate the delivery of hospice services, including nursing care, personal care, medical equipment and supplies, medications, counseling, and therapies.
  • Facilitate communication and collaboration among team members to ensure seamless care delivery.
  • Arrange for additional support services as needed, such as respite care or volunteer assistance.

Monitoring and Reassessment:

  • Regularly monitor the patient's condition and response to treatment.
  • Conduct ongoing assessments to identify changes in the patient's needs and adjust the plan of care accordingly.
  • Communicate any changes or concerns to the interdisciplinary team and involve the patient and family in decision-making.

Providing Education and Support:

  • Educate the patient and family about the hospice philosophy of care, the expected trajectory of the illness, and available support services.
  • Offer emotional support and counseling to help patients and families cope with the challenges of terminal illness and end-of-life care.
  • Provide guidance on practical matters such as caregiving techniques, medication management, and advance care planning.

Ensuring Comfort and Dignity:

  • Prioritize the patient's comfort and dignity throughout the care process.
  • Implement strategies to manage pain and other distressing symptoms effectively.
  • Respect the patient's autonomy and choices regarding end-of-life care decisions.

Documentation and Evaluation:

  • Maintain accurate and up-to-date documentation of the patient's care, including assessments, interventions, and outcomes.
  • Evaluate the effectiveness of the care plan and make adjustments as needed to optimize outcomes and quality of life.
  • Ensure compliance with regulatory requirements and standards of practice.

Possession of a current license to practice as a registered nurse in the State of California Minimum of two (2) years professional experience as a Registered Nurse.

Job Type: Full-time

Pay: $48.00 - $52.00 per hour

Expected hours: 35 – 40 per week
